Addiction Psychiatry for Behavioural and Substance Misuse

Behavioural Addiction, such as Pathological Gambling; Compulsive Eating Disorder; Compulsive Spending and Shopping;  and Chemical and Substance Misuse and Addiction – are serious mental health conditions. I and many physicians and clinicians, consider them to be illnesses. It’s not the individuals fault for having an illness and health condition; but it is their responsibility to do something about it.

Addiction invariably impacts an individual’s life significantly, and ‘Addiction’ is known to be a “Family Illness” too. That comes as a shock to many loving parents in ‘active addiction’ who are massively impacting their partner, sons and daughters, siblings and parents’ lives – not to mention the jeopardy to their own health.

It is estimated that 1 in 4 NHS admissions to a General Hospital, are Alcohol related. There are 60+ medical conditions exacerbated by Dependent Alcohol Use, such as Fatty Liver Disease, Cirrhosis, Heart Disease, Pancreatitis, Type 2 Diabetes, Stroke Disease and Alcohol Related Dementia, to name just a few. It is now estimated that Dementia is the most common cause of death in women in old age; and the second most common cause of death in elderly men after Heart Disease. You don’t want to accelerate the chances of Dementia. Chemical Addiction is like an accumulating, self-induced brain injury.

Compassionate and Evidence-Based Treatment

As a Consultant Psychiatrist, with a lot of experience working in Addiction Psychiatry – I can offer compassionate and evidence-based treatments to help people manage their Addiction and Substance Misuse. I can no longer offer emergency treatment, but can refer to colleagues in residential treatment centres for those with severe Dependence in crisis.

It was particularly sad, when an excellent residential treatment centre, The Manor Clinic in Southampton; was closed on 31 December 2022, by the company owner. Other outpatient services and Residential treatment facilities do exist in Bournemouth, Southampton and Portsmouth areas.

Following the closure of The Manor Clinic, I no longer actively oversee inpatients in residential treatment centres – but do work with patients to develop personalised treatment plans on their recovery journeys’, back to health.

Whilst some are wary of the ‘12-Step model’ of working in Addiction; programs such as Alcoholics Anonymous AA, and Narcotic Anonymous NA, can be very helpful supports – and ways to feel ‘connected’, back in your real world. Undertaking recovery from addiction is always helped by support from one’s ‘family’ whatever form that takes. Groups such AA and NA help many with making their recovery successful too.

Psychiatry provides evidence-based treatment strategies alongside individual therapy and supports such AA and NA; for those that find them helpful. It is also crucially important, in my view, to identify “Dual Diagnosis” conditions – such as Complex Trauma, PTSD (Post Traumatic Stress Disorder), Adult ADHD / ADD and Bipolar Disorder; which makes chemical addiction so much more likely in the first place. The individual needs an accurate diagnosis of all relevant conditions, to work out an effective recovery pathway back to health.

Understanding Addiction and Substance Misuse

Addiction and substance misuse are complex conditions that involve physical and psychological dependence on substances such as alcohol, drugs, or medications. These conditions can cause severe physical and psychological harm, and can significantly impact an individual’s quality of life.

Diagnosis and Assessment

To diagnose addiction and substance misuse disorders, I will conduct a comprehensive assessment, including an evaluation of the patient’s symptoms, medical history, and psychological testing. It is essential to obtain a formal diagnosis to determine the best course of treatment.

Treatment Options

Addiction and substance misuse disorders are treatable. The treatment options for addiction and substance misuse disorders include medication-assisted treatment, psychotherapy, or a combination of both.

Medication

Medication-assisted treatment (MAT) can be effective in managing Addiction and Substance Misuse symptoms.

Medication such as Acamprosate, Nalmefene and Naltrexone can help to reduce cravings and withdrawal symptoms; in Alcohol Dependence.

Formerly, it was possible to undertake community detoxes in people’s homes; from Alcohol using benzodiazepines such as Librium (chlordiazepoxide) or rarely Diazepam, in those with liver problems. This is no longer possible on an outpatient basis, as detoxing from Alcohol is a condition requiring admission to a General Hospital or undertaken in certain residential detox. Facilities where there is continuous supervision.

Psychotherapy

Psychotherapy helps to improve coping strategies, problem-solving skills, and self-awareness. Cognitive-behavioural therapy (CBT) and motivational interviewing (MI) are evidence-based treatments that can help individuals manage their addiction and substance misuse symptoms effectively.

Lifestyle Change

Lifestyle changes can also help individuals manage addiction and substance misuse symptoms. These include regular exercise, maintaining healthy nutrition, getting adequate sleep, and minimising stress.

It is fairly common that those with Substance Misuse can have a co-existing ‘Eating Disorder’; which again requires specialist intervention; usually with NHS or Independent Outpatient clinics or residential treatment.

I offer personalised services and flexibility to choose in-person or remote sessions via video call software. There are some advantages to an initial consultation being in-person; though geography or circumstance mean that video-calls are an increasingly popular format.

I will work closely with you to determine the best options for achieving your desired outcomes based on your personal and work commitments, travel restrictions, and preferences.

These days, outpatient work is limited to those who are able to maintain some social and occupational functioning. For those with Severe Dependence Syndrome, there remains a vital role for Residential Treatment. Accessing such services is difficult and invariably expensive with Independent Providers. Accessing NHS or Local Authority funding is now very time consuming and requires great patience, as fewer places exist within NHS sponsored services.

As ever, I believe it important to identify any Secondary or “Dual” Diagnoses which predispose people to developing addiction; such as PTSD, Adult ADHD, or Bipolar Disorder.
